The required evidence will either be

• • • An NZQA Record of Achievement showing completion of the following unit standard: 17588 – Apply for, accept, & carry out work according to a work permit in the workplace
    • Downer Attendance sheet showing Permit Receiver
• Where assessor / trainer signature section is shown on the evidence, this must be completed
• Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Copy must be clear and legible
• Can be in colour copy OR black and white copy

Underground Services.Licence.BeforeUdig

The evidence will be a:

• • Certificate showing beforeUdig Certified Locator

• Evidence will be issued by beforeUdig Certified Locator ONLY
• Course Completion date must be visible
• Where assessor /signature section is shown on the evidence, this must be completed
• Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Copy must be clear and legible
• Colour copy or black and white copy is acceptable

Where the evidence completion date exceeds 2 years, an additional certificate showing beforeUdig Certified Locator MUST be provided
