External.Qualified.EXT031 Turbine Licence | - Name on licence to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
- Must be a High Risk Work Licence card from state Government/Worksafe
- Licence number to be displayed
- Class to be displayed to be 'TO'
- Issued and expiry date to be displayed
- Both Sides of the licence (Front and Back) must be included within the uploaded file.
- Black and white copies can be accepted but is not preferred
- Scanned document to be clearly legible
- It is 'in date' and not expired
Exemption Process: An exemption may be granted for a training competency requirement listed in the Training Matrices for an employee or contractor. Exemptions shall be applied only after careful consideration and risk assessment. Yara Employee applications for training exemptions shall be approved by the relevant Department Manager. Contractor applications for training exemptions shall be approved by the relevant Yara Responsible Person. To be deemed valid the request shall be: - Documented on Yara’s Training & Competency Exemption Form [TRG-YP-FRM-125-09]

- Shall be ‘approved’
- Issue date: Use date of issue
- Expire date: As listed on the form
- Shall have approver role, name and signature.
