Site - Pre-Approval form / Authority to undertake induction form
  • The form must be dated and must be signed by the WHC contract owner / coordinator
  • Must be signed by relevant WHC manager if trade is being used
    Example: Manager Mechanical Engineering or Manager Electrical Engineering
  • If going underground: Must be signed by manager mining engineering

Whitehaven Open Cut:

Whitehaven Underground:

  • Admin. Letter of competency induction authorisation

 
Access keys to be assigned: Check the Authority to Undertake Induction form. For each site selected they should have an access key.
