Generic.Govt Licence/Authority.Proof of Identity

• Name on the document to match the registered person
• Must be an Australian Government issued document (birth certificate, 18 plus card, licence)
• Must be legible

• The Proof of Identity must be a State or Federal Issued Proof of Identity.  Examples include:

• Drivers Licences

• Passports

• Proof of Identity cards etc.

•  Only issue date to be recorded
