| Medical.Assessment.CS Order 41 | - Name on medical to match the person registered
- Doctors signature and date and stamp mandatory
- The certificate of fitness page to be uploaded as a minimum unless a restriction is detailed whereby the full medical will be required to be uploaded
- This document is only mandatory for specific sites if the document is mandatory the company needs to upload an Order 41 Coal Board Medical (preferably Coal Services Health).
- Medical will expire 3 years from date of examination
IMPORTANT - Fit for Duty comment If the medical is marked as ‘AMBER’, the individual must also complete the MACH Energy Worker Fitness for Work Personal Management Plan - The Order 41 report and the Management Plan must be scanned and loaded as one pdf file
- All AMBER medicals will be reported to MACH Energy
- If the Management Plan is approved, the individual will receive an invitation to complete their Online Induction
- If the Management Plan is not approved, the individual will be contacted by a MACH Energy representative
| - Issue Date = date of examination
- Expiry Date = 3 years (unless a review date is listed then enter the expiry as the earliest expiring date
- Please enter the name of the doctor in the Declaration Comment box
- All AMBER medicals must be reported to MACH Energy contact prior to validation
- Validation will not occur until written approval has been received
