• Business Name needs to match. If it doesn't match, government certified evidence must be provided to link the business names together. This is ideally a letter or download from ABR showing the linkage

• We can accept ‘PTY LTD’ or ‘(Pty) Ltd’  in lieu of ‘Proprietary Limited’ when this is the only difference in the company name between what’s shown on company compliance paperwork verse what’s shown in onsite.

• Must be the BHP Mt Arthur Coal Prequalification Approval Form (please refer to the example document). This will be issued to you from BHP Mt Arthur Coal and you must save the email as a PDF and upload as authorisation to register in this pre-qualification process.

Quality Management Policy

• Letter on Company letterhead
• Must be signed within the last 12 months.

Signed by Senior manager
A seniors manager includes:

• CEO
• CFO
• Director
• Operations Manager
• Safety Manager

Issue date = as per policy (must be signed within 12 months prior to submission

Expiry date = Not Required

Comply with Mt Arthur Coals Safety Management System

• Document must be on their Company Letterhead with the correct ABN number
• Company to use content as outlined in the template. No change to wording.
• The letter should be signed by a senior manager of the business stating their name and position
• Dated with in the past 12 months
• One pdf document per approval

Issue date required (must be dated within 12 months of date of submission)

Expiry date not required

Operate under my own Safety Management System

• Document must be on their Company Letterhead with the correct ABN number
• Company to use content as outlined in the template. No change to wording
• The letter should be signed by a senior manager of the business stating their name and position
• Dated with in the past 12 months

Issue date required (must be dated within 12 months of date of submission)

Expiry date not required

Mt Arthur Coals SMS with approved Exceptions

• Document must be on their Company Letterhead with the correct ABN number
• Company to use content as outlined in the template. No change to wording
• The letter should be signed by a senior manager of the business stating their name and position
• Dated with in the past 12 months
• Must include sign off by BHP Mt Arthur Coal Ross Carlson
• Provide copies of procedures and risk assessments
• One pdf document per approval

Issue date required (must be dated within 12 months of date of submission)
