• Contractor Classification must be ticked and must match class in Onsite.
• Duration of work, cost, & people must be completed
• Contractor management section all questions completed
• Notifications to relevant WHC department - optional
• Approval – must have name printed, must be signed and dated. Must be completed in accordance to the site listed in the field – Site/Area on this form.
• Whitehaven Coal Contractor Engagement Form 1 to be completed and uploaded
• Black and white or colour copy is accepted

• Expiration to be recorded as Issue date plus duration of project (can be found in the responses to the questionnaire)3 years from starting date except if stated otherwise in the document.

Whitehaven Coal Sub-contractor Engagement Form 5 (Subcontractor engaged by a primary contractor)

• Contracting company details must be displayed on form
• Contracting company details must match registered company details in Onsite
• Black and white or colour copy is accepted
• Document must be fully completed and signed in all signature blocks by Contract owner, Contract coordinator and nominated contractor company supervisor. Names of people required to sign can be identified in the appropriate forms that must also be uploaded (see below)

• Issue date to be recorded as Date of document approval by WHC
• Expiration date as per Whitehaven Coal Contractor Engagement Form 1

Whitehaven Coal Sub-contractor Engagement Form 5 (Uploaded by primary contractor for subcontractors they engage)

• Forms are uploaded but not against a cert – need to be transferred to non-verified cert
• Document must be fully completed and signed in all signature blocks by Contract owner, Contract coordinator and nominated contractor company supervisor. Names of people required to sign can be identified in the appropriate forms that must also be uploaded (see below)

• Issue date to be recorded as Date of document approval by WHC
• Expiration date as per Whitehaven Coal Contractor Engagement Form 1

• Document must be fully completed and signed by all required WHC personnel
• If company has selected Narrabri as the site they are working at then all three signature bloclks must be signed. Contract owner / coordinator names can be identified in the appropriate forms that must also be uploaded (see below)
• If the company has selected only Open Cut site(s) then only the Open Cut signature block needs to be completed.

• Contractor Company Acknowledgement must be completed – the Class must match FORM 1, name needs to be inserted in statement.
• The class must be selected, to confirm that they agree to classification given
• Contractor company supervisor – Must have name printed, signature and dated.

• Whitehaven Coal H&C Document Form 2 to be completed and uploaded
