  • Document must be a Statement of Attainment
  • UETTDRRF06B and HLTAID001 to be listed (version A, B, C etc. acceptable)
  • Issued by an RTO with the Nationally Recognised Training logo
  • Duration = 12 months
  • Must be clearly scanned and legible
  • Black and white or colour is acceptable
  • Issue date: As listed on the certificate
  • Expiry date: 1 year from the issue date
