    External.Course.EXT095 . Construction White Card

    Name on licence/card to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable)

    Must be a OHS Construction induction card card from state Government/Worksafe

    Licence/Card number to be displayed

    Only the front side of this card is required to be uploaded.

    Black and white copies can be accepted but is not preferred

    Scanned document to be clearly legible

    A confirmation letter or form that shows evidence of having ordered a replacement card issued by the Government/Worksafe will be accepted

Exemptions shall be applied only after careful consideration and risk assessment. Yara Employee applications for training exemptions shall be approved by the relevant Department Manager.     Must be either:

    • A statement of attainment or card issued by RTO showing either:
        • CPCCOHS1001A
        • CPCCWHS1001 or;
        • CPCWHS1001

    OR

    • An OHS Construction induction card from Australian state Government/Worksafe. Only the front side of this card is required to be uploaded

    OR

    • A confirmation letter or form that shows evidence of having ordered a replacement card issued by the Government/Worksafe will be accepted

    OR

    • The Digital version of the NSW White Card can be accepted

    OR

    • A database extract for Construction White Card


    For each of the above:

          • Name on licence/card to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
          • Licence/Card number to be displayed on all except SOA
          • Black and white copies can be accepted but is not preferred
