  • Acceptable evidence can be in the form of:

    • Certificate

    • Refresher Training

    • Trade License

    • On-the-Job Training (OJT) Documentation

    • Proof of Course Completion

  • Evidence must state completion of Electrical Safety. Some samples of acceptable course wording include (but are not limited to):

    • NFPA 70E evidence is acceptable

    • Electrical Safety

    • Electrical Safety General Awareness

    • Electrocution Prevention

    • General Electrical Hazard Awareness for Site Safety

  • Instructor Name

  • Employee Name

  • Date of training
