Name on assessment form must match applicant name (shortened version of name is accepted eg. Chris for Christopher)

The applicant's name on the document must match their registered name in the portal.
Section 1: Must include the applicant, contractor company supervisor, and approver.
Sections 2 & 3: Must be fully completed, with competencies and experience relevant to the selected role documented.
A manager/SME from the engaging company must review the supporting documents and sign before submission to Alstom for approval.
Section 5: Must be initialed by the worker.

Contractor Note:

Section 4: Requires three signatures (Contractor company supervisor/Manager/SME, Alstom Manager, and Alstom Training & Competency Manager) before uploading the document to the portal. If Alstom's signature is missing, the Contractor Admin must email the assessment to ANZ.Training@alstomgroup.com for approval.
