- Booking confirmation must include the following:
  - Specify booking/registration enrolment for either:

      - SMIC Pre-Cast training – two [2] days
      - Civil Construction (Intro to Pre-Cast Fabrication) 2 Days Fee for Service (commercial rate)
  - Booking date for all two [2] sessions are required (NOTE: Booking date must be in the future)
- Black and white or colour copies of certification is accepted
- Must be clear and legible

Expiry Date to be listed as the course start date (i.e. day 1 of training)

SMIC Pre-Cast - Competency Placeholder (Internal use only)
