- Name of inductee must match registering person
- Position must be completed
- Contractor Company must be completed
- WHC Contract Owner or Coordinator email – NOT MANDATORY
- Must select Primary WHC site and all sections must be completed.
- Induction(s) required – must be completed
- Approval section: must at minimum be signed by a contract owner / coordinator
- In addition, tradesperson roles must be signed off by a person on the authorised approvers list relevant to the role e.g. Electrical Tradesperson signed by Manager Electrical Electrical Engineering.

The Manager of Mining Engineering (MME) or CHPP Manager need to sign for the role 'All Open Cut - Operator'.
