  1. Documented on Yara’s Training & Competency Exemption Form [TRG-YP-FRM-125-09]
  2. Participant details to match applicant
  3. Course Code and Title to match
  4. Shall be ‘approved’
  5. Issue date: Use date of issue
  6. Expire date: As listed on the form
  7. Shall have approver role, name and signature
