• Must be certificated AS/NZS 4801 or OHSAS 18001 or ISO 45001:2018 
• JAS-ANZ stamp to be present on

• 4801 - If a certificate is supplied that is a combined 4801 and 18001 there will be NO JAS-ANZ stamp present. This is acceptable.

From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001 

From September 30, 2021: OHSAS 18001 will no longer be accepted - ISO 45001 MUST be supplied

• The Principle Contractor Representative must circle the relevant company, print name and sign.

• A Secondary Principle Contractor is not mandatory
• Principle Contractor signatures do not need to be verified
• Issue date – date of verification
• No expiry date

• At least one line must be completed.
• The respondent must print their name, sign and date the form.
