• Inductee name and date of birth to be clearly shown on the medical and must match the details in the system
• Must be stamped and signed by the medical practitioner
• Must be a JobFit or MRM medical form
• Must have NO RESTRICTIONS outlined on the certificate

• Issue Date: date of examination by the EMO (EMO date not the NMA sign off date)
• Expiry Date: Two (2) years from the date of examination UNLESS there is a review date which then becomes the end date
