Health Safety Management Plan
| - A Contractor Safety and Health Management Plan (SHMP) must be completed for all contractor companies requesting registration for Cracow Mine – Please click HERE for a copy of the Form
- A SHMP confirms that a contractor company will comply with the Cracow Safety and Health Management System (SHMS) while operating on site at Cracow.
- Contractor companies are to replace or delete all green text as applicable to their company and scope of work at Cracow.
- Section 7 of the template provides access to the Cracow SHMS – these documents are to be complied with while operating on site.
- If the contractor company wishes to utilise its own procedures, safe work instructions or other documents while on site, they must submit these documents for approval as part of their SHMP – Section 7.2 is required to be completed in these instances.
- Once completed, the signed SHMP form is to be submitted to your Responsible Contract Owner (RCO) for site review and approval -If you are unsure of your RCO, please contact your Aeris Site contact for Guidance
- Once approved, you will be returned the signed SHMP for uploading to Pegasus.
Pegasus Acceptability Criteria - All green sections of the SHMP must be complete
- Section 9 - Must be signed off by Contractor Representative
- Section 10 - Must be signed off by The Cracow RCO
- Section 11 - Must be signed off by The Cracow SSE
