Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable): Acceptable evidence can be in the form of:

• Certificate

• Refresher Training

Evidence must state completion of Bucket Truck Color copy or black and white copy is acceptable

• Issue Date = Date of completion

• Expiry Date = As specified on the evidence. If not specified: Input expiry as 3 years from date of completion

Health & Safety.Permit to Work.Aerial Lift Operator Training

• Acceptable evidence can be in the form of:

  • Certificate

  • Refresher Training

  • License

• Evidence must state completion of Aerial Lift Operator or similar (such as Mobile Elevated Work Platform, Boom Lift, Scissor Lift)

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Colour copy or black and white copy is acceptable 

• Issue Date = Date of completion

• Expiry Date = 3 years from date of completion

Health & Safety.Permit to Work.Equipment Operator

• Acceptable evidence can be in the form of:

  • Certificate

  • Refresher Training

• Evidence must state completion of Heavy Equipment Operator

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Colour copy or black and white copy is acceptable 

Valid for the period of time stipulated by the issuing entity.

Health & Safety.Permit to Work.Powered Industrial Truck (PIT)/Forklift Operator

• Acceptable evidence can be in the form of:

  • Certificate

  • Refresher Training

  • License

• Evidence must state completion of Powered Industrial Truck (PIT) or Forklift Operator

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Colour copy or black and white copy is acceptable 

• Issue Date = Date of completion

• Expiry Date = 3 years

Health & Safety.Permit to Work.Rigger Certification

• Acceptable evidence can be in the form of:

  • Certificate

  • Certificate in format of card / licence

  • Refresher Training

• Evidence must state completion of Rigging / Rigger

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Colour copy or black and white copy is acceptable 

• Issue Date = Date of completion

• Expiry Date = 2 years

Health & Safety.Permit to Work.Rigger Qualification

• Acceptable evidence can be in the form of:

  • Certificate

  • Refresher Training

  • License

• Evidence must state completion of Rigging / Rigger (Qualified)

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Issue Date = Date of completion

• Expiry Date = 2 years

• Evidence must state completion of Trenching & Excavation Competent/Qualified Person
