• Photo of the form can be accepted in lieu of actual scanned copy providing all information can still be clearly seen

• RIIRIS301 Apply risk management processes
• RTO Name, National Provider Code / RTO Number & Logo and Nationally Recognised Training (NRT) logo to be displayed on document
