Introduction to Process Safety at Yara (INT001)	This competency may be completed after commencement of employment Yara Pilbara Training Attendance form [TRG-YP-FRM-125-02] with matching internal course competency and code Participant Details to match applicants profile. Note: Shortened versions of first names are accepted, such as: "Chris" instead of "Christopher" & "Wil" instead of "William“ Note: if the name is not clear or cannot be confirmed the Yara ID number &/or Company Name may be used to verify identity Applicant's signature Authorised Trainers complete name and signature	Issue date = Use the date of training delivery Expiry date = 5 years from issue date
