Enter & Work in Confined Spaces	Name on certificate to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable) Document must be a Statement of Attainment Statement of Attainment Enter and Work in Confined Spaces to be listed Competency unit/qualification code must match exactly Must be Issued by an RTO with Nationally Recognised Training logo Black and white or colour is acceptable Scanned document to be clearly legible Note: If a card is supplied there must be a copy of the front and back displaying the units of competency, person's name, issue date as above and must be issued by an RTO An exemption may be granted for a training competency requirement listed in the Training Matrices for an employee or contractor. Exemptions shall be applied only after careful consideration and risk assessment. Yara Employee applications for training exemptions shall be approved by the relevant Department Manager. Contractor applications for training exemptions shall be approved by the relevant Yara Responsible Person.
