Mining.Induction.Standard 11 - Surface Operations	Duration = 5 years Can be either the Statement of Attainment Certificate or the STD 11 Card The card must be scanned front and back Is statement of completion acceptable? Do all 6 units have to be listed?		Duration = 5 years* Name on certificate to match the person registered Must be a statement of attainment* Black and white or colour acceptable Does it have to be issued by a certain provider? All 6 of the following units must be listed: RIICOM201A: Communicate in the Workplace RIIGOV201A: Comply with site work processes/procedures RIIOHS201A: Work safely and follow OHS policies and procedures RIIRIS201A: Conduct local risk control RIIERR205A: Apply initial response First Aid RIIERR302A: Respond to local emergencies and incidents Letter of end of each RII may differ from ‘A’ – this is accepted (e.g: RIICOM201B) *Statement of Completions are acceptable for 3 months from the date of issue (3 month expiry from issue to be recorded)	Issue Date = date issued Expiry Date = as per site duration stipulated Must be clearly scanned and legible
Resource Industry.Certificate of Competency.Standard 11 - Surface Operations - PARTIAL COMPLETION	Must be a certificate of Registration with a mining endorsement unless it is a re-issue, in which case receipt of payment will suffice Must be issued by Surveyors Board Queensland			Issue Date: As per the issue date identified on the registration. Expiry Date: One (1) year from the identified issue date.
Generic Underground Induction		Name to match Issued within the last 12 months Issued by Coal Service Mines Rescue only All 67 of the following must be listed:- RIICOM201D Communicate in the workplace RIIERR203D Escape from hazardous situation unaided RIIEII205D apply initial response first aid RIIERR302D Respond to local emergencies and incidents RIIGOV201D Comply with site work processes/procedures RIIRIS201D Conduct local risk control RIIWHS201D Work safely and follow WHS policies and procedures		Duration = 5 years Issue Date = date issued Expiry Date = as per duration unless stipulated
Resource Industry.Statement of Attainment.Standard 11 - Underground Operations	Can be either the Statement of Attainment Certificate OR the STD 11 Card (Green Card or 'issued on achievement of competence in:') All 7 of the following units must be listed: RIICOM201, RIIGOV201, RIIOHS201 (now replaced by RIIWHS201), RIIRIS201, RIIERR205, RIIERR302 and RIIERR203			Issue Date: as per the issue date identified on the Statement of Attainment or the ID card Expiry Date: five (5) years after the issue date identified
