• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
• Document must be a Statement of Attainment
• Statement of Attainment must show Gas Test Atmospheres
• Must be Issued by an RTO with Nationally Recognised Training logo
• Black and white or colour is acceptable
• Scanned document to be clearly legible

Note: If a card is supplied that meets all requirements but states 'Information contained in this card is for student reference only', this is acceptable

• Issue Date to be entered as date of training completion
• Expiry date: 3 years from the issue date

    Must show that the person is competent and authorised to operate
