  • Must be completed by an approved medical practitioner as per the link below

  • Order 43 Preplacement Medical Assessment Report to be uploaded
  • ALL SECTIONS MUST BE COMPLETED – Full medical not required - Certificate of Fitness only to be uploaded (Traffic light page)
  • Medicals completed after the 1st July 2018 must be Order 43
  • Name on medical to match the person registered
  • Date of birth to match person registered
  • Employer must be listed
  • SEG completed
  • Date of assessment listed
  • Determination – must be ticked indicating result
  • Does not need to be in colour – black & white acceptable

 

  • IMPORTANT – Restricted / AMBER medicals

If the medical result being submitted is AMBER, the company must supply a declaration on company letterhead stating that:

 

  • Must be presented on a company letterhead
  • The medical has been approved by a senior company representative and must be signed by a senior company representative, e.g. supervisor, HR representative, return to work coordinator
  • The employee is fit for duty to perform tasks whilst on the MACH Energy site under the medical restriction.
  • State that the employee’s medical has been approved by the company
  • Be dated within the last 3 months

 

•If the medical submitted is RED then the document is not to be approved and must be returned to the company.

 

  • Chest X-ray must be ticked yes or no
  • Must have either Doctors signature or Registered Nurse signature, must have name, ARN number and date.
  • This document is only mandatory for specific sites (NSW)
  • Must be uploaded as a PDF

 

Section 4 Queensland Coal Board medicals are NOT accepted
  • Issue Date = Examination date
  • Expiry Date = 3 years from date of examination (unless review date listed in comments)

 

•      All AMBER medicals submitted along with a declaration by the company
Medical.Assessment.CS Order 41
  • Name on medical to match the person registered
  • Doctors signature and date mandatory
  • The certificate of fitness page to be uploaded as a minimum unless a restriction is detailed whereby the full medical will be required to be uploaded
  • This document is only mandatory for specific sites, if the document is mandatory the company needs to upload  an Order 41 Coal Board Medical (preferably Coal Services Health)
  • Part D – certificate of fitness must be accompanied by the compliance confirmation email from CS Health and must say “Order 41 Compliance status – compliant”.

    The Preplacement medical detailed certificate completed by CS Health is accepted and does not require the confirmation of compliance to be attached.

  • Medical will expire 3 years from date of examination

IMPORTANT – Restricted / AMBER medicals

  • The employee is fit for duty to perform tasks whilst on the MACH Energy site under the medical restriction.

 

    • Issue Date = date of examination
    • Expiry Date = 3 years (unless a review date is listed then enter the expiry as the earliest expiring date)
    • Please enter the name of the doctor in the Declaration Comment Box
    • All AMBER medicals submitted along with a declaration by the company; the declaration must:

  • The employee is fit for duty to perform tasks whilst on the MACH Energy or associated contractors site, under the medical restriction/s.
  • The Medical Management Controls as documented with in the Order 41 Medical have been reviewed with the employee and are listed with the company declaration.

  • Issue Date = date of examination
  • Expiry Date = 3 years (unless a review date is listed then enter the expiry as the earliest expiring date)

    All RED / AMBER / RESTRICTED medicals are to be submitted along with a declaration by the company; the declaration must:
  • Be presented on a company letterhead
  • The Medical has been approved by an authorised representative, with position and contractual right to make such approval.
  • Be dated within the last 3 months
  • State that the employee’s medical has been approved by the company
  • State that the employee is fit for duty to perform tasks whilst on the MACH Energy or associated contractors site under the medical restriction.
  • The Medical Management Controls are to be documented in accordance with the Order 41 Medical.
  • The Employee must sign to advise they will comply with the restriction/s.

ORDER 43 MEDICAL

  • Must be completed by an approved medical practitioner as per the link below if completed by an external provider (Blue cover page)

  • ALL SECTIONS MUST BE COMPLETED           
  •  Must be uploaded in colour
  •  If the cover page is orange – medical was completed by CS Health and does not require the SEG number and does not need to be a doctor on the approvers list
  • If the cover page is blue – medical was completed externally and MUST have SEG number 
  • Medicals completed after the 1st July 2018 must be Order 43
  • Name on medical to match the person registered
  • Date of birth to match person registered
  • Employer must be listed
  • SEG completed – (not required for CS Health medical (Orange cover page) 
  • Date of assessment listed
  • Determination – must be ticked indicating result
