•   Must be the "Peabody Open Cut Core Induction - Core Modules"  used across the Burton, Coppabella and Moorvale platforms.

• Issue Date: The completion date identified on the certificate
• Expiry Date: Five (5) years after the issue date.

Site.Induction.Coppabella - Induction Approval Form

• Must be Coppabella Induction Request Form
• Name must match person registered
• Induction date to be listed on document
• Induction title / purpose to be listed on document
• Portal role must be selected

• Must be signed by Coppabella Department Manager

• Issue date to be recorded - Date of manager signature
• 5 years from date of manager signature
• Must be clearly scanned and legible

• Must be the previously site specific "All Coal Mine Worker Induction" used at Coppabella

• Issue Date: The completion date identified on the certificate
• Expiry Date: Five (5) years after the issue date.

•  Must be the "Peabody Opencut Core Induction - Coppabella Site SOPs" or the previously site specific role packages used at Coppabella

• Issue Date: The completion date identified on the certificate
• Expiry Date: Five (5) years after the issue date.

• Must be NSW Peabody Induction Request Form
• Name must match person registered
• Portal role must be selected

• Must be signed by Metropolitan Department Manager

• Duration = 24 months
• Issue Date = date signed
• Expiry Date = as per duration unless stipulated

• Name to match
• Issued within the last 12 months
• Issued by Coal Service Mines Rescue only
• All  7 of the following must be listed:-
  • RIICOM201D Communicate in the workplace
  • RIIERR203D Escape from hazardous situation unaided
  • RIIEII205D apply initial response first aid
  • RIIERR302D Respond to local emergencies and incidents
  • RIIGOV201D Comply with site work processes/procedures
  • RIIRIS201D Conduct local risk control
  • RIIWHS201D Work safely and follow WHS policies and procedures.

• Duration = 5 years
• Issue Date = date issued
• Expiry Date = as per duration unless stipulated

• Name to match
• Issued by Peabody within the last 24 months

• Duration = 24 months
• Issue Date = date signed
• Expiry Date = as per duration unless stipulated

• Name to match
• Issued by Peabody within the last 24 months

• Duration = 24 months
• Issue Date = date signed
• Expiry Date = as per duration unless stipulated

• Name to match
• Issued by Peabody within the last 12 months

• Duration = 12 months
• Issue Date = date signed
• Expiry Date = as per duration unless stipulated

Site.Induction.Millennium - Induction Booking Form

• Name must match person registered
• Induction date to be listed on document
• Induction title / purpose to be listed on document

•  Issue date to be recorded
• Check the 3 authorisation sections have been signed off

• Must be the Moorvale Induction Request Form
• Name must match person registered
• Induction date to be listed on document
• Induction title / purpose to be listed on document
• Portal role must be selected

• Must be signed by Moorvale Department Manager

•   Must be the "Peabody Opencut Core Induction - Moorvale Site SOP'

• Issue Date: The completion date identified on the certificate
• Expiry Date: Five (5) years after the issue date.

• Check the 2 authorisation sections have been signed off – requester and 1 of either Wambo job coordinator or department manager

• Induction Request form signed by one of the following site contacts – it is the contracting companies responsibility to get the form signed – do not approve booking without signed approval
