SMS 4801.Safety Management.AS/NZS4801 OR ISO48001 Certificate OR ISO45001:2018	Business Name on Certification to match the organisation registered. If it doesn't match, government certified evidence must be provided to link the ABN or business names together. This is ideally a letter or download from ABR showing the linkage. Must be certificated AS/NZS 4801 or OHSAS 18001 or ISO45001:2018 JAS-ANZ stamp to be present on 4801 OHSAS 18001 is International and therefore must be issued outside of Australia From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001 From March 12, 2021: OHSAS18001 will no longer be accepted - ISO 45001 MUST be supplied Issue and expiry as per certification Black and white or colour is acceptable	Issue and expiry as per certification
Safety Management System Sub-contractor Authority Form	All fields to be completed Principal Contractor and Authorized Company Representative signatures to be present with dates	Dates will automatically align with SMS certification
Resource.Contractors.Sedgman Sub-contractor Authority Form	All fields must be completed The sub-contractor company name must match the registering company in the system. The Authorised Company Representative from the Principal Contractor, must complete the form and sign and date The Authorised Company Representative from the sub-contractor company, must sign and date	Issue date – Date signed by principle company Expiry date – 2 years from issue date
Resource.Contractors.Thiess Sub-contractor Authority Form	All fields to be completed Thiess authorised representative must print name and sign Authorized Company Representative must print name and sign The following Thiess personnel are able to approve: Michael Redman Michael Godschalk Ryan Fox Peter York Ian Edwards Sherelle Hendriks Beth Viertel - Authorised until Friday 13.03.2020	Dates will automatically align with SMS certification
