Working at Height.Statement of Attainment.BCGCM1006A Work safely at heights	Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable) Document must be a Statement of Attainment Statement of Attainment must show BCGCM1006A Work safely at heights Must be Issued by an RTO with Nationally Recognised Training logo Black and white or colour is acceptable Scanned document to be clearly legible	Issue Date to be entered as date of training completion Expiry date to be entered as 2 years from the issue date
