Waste Transport Licence (NSW)	If a Sole Trader or Trust the name on Licence should match the person registered If a Partnership: the name on the Licence should match one of the Partners name registered in the system If a Company: the name on the licence should match the companies registered name Waste Management Licence to be listed Colour copy required Front and back required Must be issued by the relevant states Government appointed organisation and the organisation logo must be present	Issue Date = Start date if displayed or enter as yesterdays date Expiry Date = as shown on Licence

Controlled Waste Carrier Licence (WA)	If a Sole Trader or Trust the name on Licence should match the person registered If a Partnership: the name on the Licence should match one of the Partners name registered in the system If a Company: the name on the licence should match the companies registered name Environmental Protection (Controlled Waste) to be listed Colour or black and white colour acceptable Must be issued by Government of Western Australia and the organisation logo must be present	Issue Date = Commencement date as listed on licence certificate Expiry Date = as shown on Licence
