Letter of Competency must: 

  • Be completed on the Thiess Letter of Competency template

Applicant Details

  • Must be FULLY completed

Qualifications and tasks to be completed during time on site

  • A list of qualifications and tasks to be completed during time onsite

Employment Details

  • Must be FULLY completed

Details of experience in tasks to be performed onsite

  • Must be listed

All experience in the LOC must be specific to work on Australian sites

Details of training that has been provided by the employer with regard to work procedures, risk management, hazard management, the company's WHS safety management system and employee obligations

  • Must be listed

Equipment where maintenance experience exists for the applicant

  • Electrical/Mechanical Only – Must be listed

Applicant performance relating to accidents or incidents

  • Must be listed. List as NA if applicable

Date/s of current NSW Order 41/41 medical & Chest X-Ray

  • Must be listed

Level of Supervision required for applicant

  • Must be listed

Trainee Only - List the applicant’s current Training Modules

  • Must be completed
  • A Trainee role must detail level of supervision required for applicant plus include a list of current Training Modules
  • An Apprentice role must detail level of supervision required for applicant

Electrical/Mechanical Only - Will Hot Work be performed by applicant? 

  • Yes/No If yes, supply MDG25 Awareness Training qualification - MUST SELECT HOT WORK ROLE - Pegasus to check role has been selected.

Verifiers Details – must be a Senior Manager

  • Must be FULLY completed
  • Must be signed by senior manager of company. 'Chief Operating Officer or CEO or Director or Operations Manager' are acceptable  
  • Electronic signature can be approved during covid
  • Issue Date = date on the letter (must be dated within 1 month at the time of submission
  • Expiry Date = no expiry date to be recorded
