Health & Safety.Course.MSHA Part 46 Annual Refresher Training

  • Acceptable evidence can be in the form of:

    • MSHA 5000-23 Form

    • Written document

  • Evidence must state Part 46

  • Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

  • Colour copy or black and white copy is acceptable 

 

If a written document is provided, it must include the following information:

  • Display company letterhead

  • Full name of the applicant

  • Type of training

  • Duration of training

  • Date the training was received

  • Location of training

  • Name and signature of the competent person who provided the training

  • Must include the statement “False certification is punishable under section 110 (a) and (f) of the Federal Mine Safety and Health Act”

  • Issue Date = Date of completion

  • Expiry Date = 1 year
