• Name on certificate to match the person registered (Shortened version of name is accepted EG. Chris for Christopher)

• Name of Orientation must match the competency

• Note: Secure Orientation = Contractor Orientation Or Company Orientation

• The names of the other orientations will be the same

• Expiry to be entered as 1 year from date shown
