Emergency Response.Course.Emergency Action Plan

• Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

• Acceptable evidence can be in the form of:

  • Certificate

  • Refresher Training

  • Safety Meeting/Toolbox Talk

• Evidence must state completion of Emergency Action Plan

• Colour copy or black and white copy is acceptable 

• Issue Date = Date of completion

• Expiry Date = 1 year
