Acceptable evidence can be in the form of:

Certificate

Refresher Training

Evidence must state completion of Hot Work Permit Training

Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

Colour copy or black and white copy is acceptable 

Issue Date = Date of completion

Expiry Date = 1 year
