Gas.Certificate of Competency.E7 Cathodic Protection Survey	The required evidence will be one of the following: Gas CoC New/Refresher Reporting form issued from a recognised training provider showing completion "C" or "tick" of Cathodic Protection Survey AND NZQA Record of Achievement showing completion of the following unit standards: US32036, 32037 OR Gas CoC New/Refresher Reporting form issued from a recognised training provider showing completion "C" or "tick" of Cathodic Protection Survey AND a copy of their Mito CoC card or MITO database screenshot showing expired Cathodic Protection Survey Where assessor / trainer signature section is shown on the evidence, this must be completed Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted). Copy must be clear and legible Can be in colour copy OR black and white copy Where the evidence completion date exceeds 2 years, an additional document showing Gas CoC New/Refresher Reporting form issued from a recognised training provider showing completion "C" or "tick" of Cathodic Protection Survey	Issue Date= Date application has been proceeded Expiry Date= 3 years following issue date
