Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

Each of the following modules must ALL be displayed on the evidence: First Aid and CPR,

Must have a hands-on portion such as American Red Cross or American Heart Association. (No online only certs will be accepted)

Certificate of completion or Card is acceptable

Issue Date = Date of Completion

Expiry Date = 2 years from issue date.

This is regardless of whether a differing validity is displayed.
