Health & Safety.Permit to Work.Current Electrical Safety (Qualified)	Acceptable evidence can be in the form of: Certificate Refresher Training Trade License On-the-Job Training (OJT) Documentation Proof of Course Completion Evidence must state completion of Electrical Safety NFPA 70E evidence is acceptable Electrical Safety Electrical Safety General Awareness Electrocution Prevention Instructor Name Employee Name Date of training Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable).
