• Must be the Mt Arthur South 'Mechanical' Appointment Form
• Must be form version 5.0
• Name on appointment form to match the person registered
• Signature accepting appointments below section - must be signed by the worker

• Appointment form does NOT require Thiess signature prior to upload.

• All sections of the form must be complete (section 5 can be incomplete)
• Section 2 for Qualified Mechanical Tradesperson must be filled or Section 3 if they are Non-statutory Mechanical Worker.
•  Both sections if they have multiple roles/appointments.
