Whitehaven Coal Sub-contractor Engagement Form 5 (Subcontractor engaged by a primary contractor)	Contracting company details must be displayed on form Contracting company details must match registered company details in Onsite Black and white or colour copy is accepted Document must be fully completed and signed in all signature blocks by Contract owner, Contract coordinator and nominated contractor company supervisor. Names of people required to sign can be identified in the appropriate forms that must also be uploaded (see below)	Issue date to be recorded as Date of document approval by WHC Expiration date as per Whitehaven Coal Contractor Engagement Form 1
Whitehaven Coal Sub-contractor Engagement Form 5 (Uploaded by primary contractor for subcontractors they engage)	Forms are uploaded but not against a cert – need to be transferred to non-verified cert Document must be fully completed and signed in all signature blocks by Contract owner, Contract coordinator and nominated contractor company supervisor. Whitehaven Coal HSC Document Form 2	Document must be fully completed and signed by all required WHC personnel If company has selected Narrabri as the site they are working at then all three signature bloclks must be signed. Contract owner / coordinator names can be identified in the appropriate forms that must also be uploaded (see below) If the company has selected only Open Cut site(s) then only the Open Cut signature block needs to be completed.
