Letters are not accepted in lieu of this. Recognised Training Institution name to be displayed on document Must be clearly scanned and legible Black and white or colour is acceptable No expiration required	Issue date: As listed on the certificate
Driver.Govt Licence/Authority.Dangerous Goods	Name on licence to match the name registered in the system Dangerous goods to be listed Must be issued by State Government body Duration as per licence Black and white or colour acceptable Must be clearly scanned and legible	Issue date: As listed on the Licence (if shown) Expiry Date: As listed on the licence
Driver.Govt Licence/Authority.Drivers Licence	Name on licence to match the name registered in the system Photo to match person Class C, HC, HR, LR, MC, MR, R, RE to be listed International licence is accepted Learner or Provisional licence is accepted Must be current Must be issued by relevant state Government authority Only require front of licence as mandatory Must be in colour Scanned document to be clearly legible Driver licence receipt showing relevant class can be accepted with a validity recorded as indicated on this form temporary evidence	Issue date: As listed on the Licence (if shown) Expiry Date: As listed on the licence
