• Details of training that has been provided by the employer with regard to work procedures, risk management, hazard management, the company's WHS safety management system and employee obligations

Electrical/Mechanical Only:

• A list of equipment where maintenance experience exists for applicant

• Applicant performance relating to accidents or incidents, list as N/A if not applicable

• Date(s) of current NSW Order 41/41 Medical AND Chest X-Ray

• Detail the level of supervision required for applicant

Trainees Only:

• List the applicant's current Training Modules

Electrical/Mechanical Only:

• Will Hot Work be performed by applicant? Must state yes or no
• If yes:
  1. Hot Work role must also be selected in Onsite AND
  2. "MDG25 Full Awareness Training" document must be provided (Expiry Date = 3 years from date of issue)
