• Lendlease form LLE1002F Approval to Supervise Assessment must be uploaded
• All pages of the 3 page document need to be included within the uploaded file
• All fields in Part A and Part B need to be complete
• Document must display lendlease Logo
• Either black and white or colour copy of document is accepted

• The Issue date in Onsite should be recorded as the date of completion shown on the certificate
• The expiration date in Onsite should be recorded as the date which is calculated to be 2 years from the issue date
