| OH&S-WHS.Induction.Site Safety Card | - Applicants name should be displayed on the evidence and should match the applicant name that's registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
- The uploaded evidence will be a site safety card showing completion of the 'Foundation passport - building' course
- Evidence will be issued by Sitesafe NZ
- Must be current
- Only require front of licence as mandatory
- Colour copy OR black and white copy is acceptable

- Documentation which has been found to be subject to alterations will not be accepted.
