NOTE: If an asbestos removal licence in a business name is submitted, the worker must be listed as one of the nominated supervisors overseeing the relevant class of asbestos removal work | - The Issue date in Onsite should be recorded as the date of completion shown on the certificate / card
- Expiry date to be listed as shown on the evidence
