The required evidence will be

• NZQA Record of Achievement showing completion of the following unit standard:
  • US17309 - Operate a wheeled loader on infrastructure works sites
• Applicant’s name should be displayed on the evidence and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Date of unit standards achieved must be visible 
• Copy must be clear and legible
• Can be in colour copy OR black and white copy

Issue Date = Credit Result date

The required evidence will be either a:
