The required evidence will be:

• • • Powerco Appendix B – Contractor Equivalency Declaration document 
• The evidence must indicate Competency class – 6D
• The evidence Must have Powerco’s Logo and match example document
• The evidence Must have PDF format
• Where “declaration completed by” and “Powerco Network Operations Manager” must both be signed and dated
• Applicant’s name MUST be displayed on the Declaration and should match the applicant’s name that is registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted).
• Declaration must be clear and legible
• Can be in colour copy OR black and white copy

Issue Date= Date signed
