| OHS & WHS.Certificate.Drug and Alcohol Screening | - Name on Drug and Alcohol Screening to match the registered person
- Must have been completed within 14 days prior to the individuals application date (not the famil date / induction date)
- Must be complete by an accredited testing facility
- D & A screening in accordance with AS4308 (urine standard) not AS4760 as this is an oral standard Urine testing shall screen for:
- Amphetamines
- Benzodiazepines
- Opiate
- Cannabinoids metabolites
- Cocaine Metabolites - Negative results must be shown to be acceptable
In the case that a positive D & A test is received laboratory confirmation that the drug is consistent with declared medication must be submitted with the D & A paperwork and will be sent to site for approval
