OHS & WHS.Certificate.Drug and Alcohol Screening
  • Must have been completed within 30 days prior to the individuals application date (not the famil date / induction date)
  • Name on Drug and Alcohol Screening to match the registered person
  • Must be completed by an accredited testing facility / collector which is checked by ensuring the document was issued by medical practice, laboratory /clinic or pathology
  • If this D & A test is completed internally, the collector MUST have an accreditation number and this must be listed on the screening form
  • D & A screening in accordance with AS4308 (urine standard) not AS4760 as this is an oral standard Urine testing shall screen for:
    -  Amphetamines
    - Benzodiazepines
    - Opiate
    - Cannabinoids metabolites
    - Cocaine Metabolites
  • Negative results should be shown to be acceptable**
  • Alcohol result must = 0.000% or 'Not Detected'

    If the Alcohol test result is greater than 0.000% then the application will be returned to the company for re-submission

  • MACH Competency Waiver Form may be accepted

Issue Date = Date of issue / test shown on results
Expiry Date = 2 years from date of issue 

**In the case that a Non Negative drug and alcohol test result is supplied, laboratory confirmation that the drug which has been detected is consistent with declared medication must be submit with the drug and alcohol test result paperwork - All paperwork must then be supported by a declaration by the company too (all documentation should be included within the one upload); the declaration must:

  • Be presented on a company letterhead
  • Be signed by a senior company representative, e.g. supervisor, HR representative, return to work coordinator
  • Be dated within the last 3 months
  • State that the employee’s drug and alcohol screening paperwork has been approved by the company
  • State that the employee is fit for duty to perform tasks whilst on the MACH Energy site despite the initial positive drug testing result.

  • Must have been completed within 30 days prior to the individuals application date (not the famil date / induction date)
  • Name on Drug and Alcohol Screening to match the registered person
  • Must be complete by an accredited testing facility / collector which is checked by ensuring the document was issued by medical practice or pathology
  • D & A screening in accordance with AS4308 (urine standard) not AS4760 as this is an oral standard Urine testing shall screen for:
    -  Amphetamines
    - Benzodiazepines
    - Opiate
    - Cannabinoids metabolites
    - Cocaine Metabolites
  • Negative results should be shown to be acceptable**

A confirmed positive drug test excludes the worker from obtaining induction and site access.
