Generic.Induction.Drug & Alcohol Test

  • Urine drug test and breath alcohol test completed within the last 30 days of the role application by a NATA accredited collector/tester
  • 0.000% BAC and Negative for drugs at levels specified by AS/NZS 4308:2008.
  • Any non-negative drug results must supply a negative confirmatory testing result performed by a NATA accredited laboratory.
  • Issue Date = Date completed to be recorded
  • Expiry Date = Not required

Medical.-.Fitness For Duties Assessment

Fitness for duties requirement:

·        Letter must include Contracting Company’s logo and Contracting Company’s supervisor/manager/HSE representative’s signature.

·        Letter must be outlined as below: EMPLOYEE is employed as a JOB TITLE at COMPANY and has been in the position for NUMBER years. In total EMPLOYEE has NUMBER years’ experience as a JOB TITLE. EMPLOYEE has been medically assessed and is currently fit to perform the work associated with this role for GM3. COMPANY will inform the GM3 representative of any change to EMPLOYEE’s fitness to undertake this work.

From 1 Feb 2020, fitness for duties competency will require a pre-employment medical to demonstrate the contractor is able to perform the inherent requirements of the role and meet the Appendix A – minimum medical requirements (IMCP0185) within the Medical Assessment Procedure. A functional capacity evaluation report will also need to be included under the fitness for duties competency.

Exemption Note: Ad-hoc contractors who complete limited work annually may continue to provide a Fitness for Duties letter following GM3 Department Manager’s approval.

Medical.-.Functional Assessment

  • Functional assessment for the role being undertaken
  • Name on assessment to match the person registered
  • Workers will undertake a role-specific functional evaluation (FCE) with a trained health professional – the worker must be found to be functionally fit at the completion of the FCE
  • Workers must meet at least AVERAGE cardiovascular fitness (completed via 3-minutes Chester Step Test)

 Order 43

Medical.-.Order 43 Coal Board Medical

  • All workers must complete Order 43 medicals to ensure the Order 43 requirement is met
  • Order 43 medicals must be conducted by Coal board approved medical practitioner. A full medical report must be attached.
  • Medicals must correctly reflect the worker’s personal details and applied role
  • Contracting company must ensure Order 43 medicals have been forwarded to Coal Service for compliance check
  • Contracting company must ensure workers meet the Order 43 legislative requirements – including requirements for Periodic Medical Assessment and Pre-Placement Medical Assessment. Random selection of audits will occur to ensure contractors meet the Order 43 compliance
  • The minimum acceptable standards for any medical concerns noted during any medical assessment(s) are shown in Appendix A – Minimum Medical Requirement 
  • Issue Date = Date of examination
  • Expiry Date = 3 years (unless Order43 medical stated earlier follow up review is required) 

Medical.-.Order 43 Coal Board X-Ray

  • Must be ILO Chest X-ray
  • Must be current and reflects worker’s name and date of birth
  • Acceptable uploaded documents include X-ray report and/or X-ray results referenced in the corresponding Order 43 medical with date of completion
  • Scanned document to be clearly legible
  • Issue Date = Date of x-ray
  • Expiry Date =3 years (unless a review date is listed then enter the expiry as the earliest expiring date
