• Must have MAC Manager and employee signatures. NOTE: Russell Wood will sometimes use an electronic signature, which is acceptable to use.
• Appointment Letter only required to be done annually if electrical and has been set to a one-year expiry date
• BHP MAC Electrical manages renewal of Appointment Form. 55

  Mechanical
• Must have MAC Manager and employee signatures 
• Examples of roles that belong to the mechanical group are: Hose Technician, Mechanical Fitter, Mechanical Boilermaker, Plant Mechanic etc. NOTE: These are only a few examples.
