Appointment Letter only required to be done annually if electrical and has been set to a one-year expiry date BHP MAC Electrical manages renewal of Appointment Form. The role selected in onsite must match the role listed in the “Select Competency Area” field on the appointment letter. If the role does not match the letter cannot be accepted and must be returned to amend role selection.	Issue Date and Expiry Date to be recorded Issue Date = Date of Letter Expiry Date = 12 months from Issue Date

Appointment form must have MAC Manager and Employee Signatures and Dated by both parties – Electronic Signatures are acceptable. The date of the appointment must be no older than 3 months.
